EXISTING PATIENT RE-CERTIFICATION

- Your in person visit with the Doctor/Physical Exam

- ALL (3) 70 day medication orders as max allowed by the State
PATIENT TRANSFER
$1997/MONTHS
$49 plus $150 for certification/orders (if applicable)
- Covers all remaining orders during your 210 day certification
- If you’re out of orders, you will need to get re-certified
- State maximum on all routes of administration
